Online Recertification Report

You must be logged in to recertify online. Please log in using your user name and password, then click "My Certification" to recertify online.

(Please note that choosing to complete the recertification report online will facilitate faster review and processing. Also note, that you must print the report and obtain the proper signatures!)

Recertification by Examination

Recertification by examination applications may be completed beginning June 15th, 2010. Examinations MUST be taken before September 15th, 2010 to be eligible for 2010 recertification. Click Here to Learn More About Recertification by Examination.
